Where it hurts tells you why
Knee pain in cyclists is overwhelmingly a fit or load problem. The knee is a hinge caught between the hip (driving the pedal down) and the foot (anchored to the pedal). When the geometry above or below is off, the knee takes the load. Pain in different parts of the knee points to different root causes.
A classic symptom map: anterior pain (around or under the kneecap) is usually patellofemoral — saddle too low or too far forward, compressing the kneecap against the femur with every stroke. Lateral pain (outside of the knee) is often iliotibial band syndrome — the IT band snaps over the lateral epicondyle, typically from a saddle that's too high, a cleat angle that's off, or a sudden spike in training volume. Medial pain is less common but usually tracks to the foot collapsing inward (pronation), which drags the knee with it.

Saddle height — the 25–35° rule
The simplest check is the knee-angle method: at the bottom of the pedal stroke (6 o'clock, crank in line with the seat tube), the knee should bend 25–35°. Less than 25° (nearly straight) and the saddle is too high — overloads the back of the knee and hamstrings. More than 35° (still very bent) and the saddle is too low — jams the kneecap into the femur, classic anterior pain.
The heel method gets you close: in cycling shoes, place your heel on the pedal at the bottom of the stroke. Your leg should be fully straight. When you clip back in (ball of the foot on the pedal), you get the 25–35° bend. Start there, then fine-tune 2–3mm at a time over several rides. The traditional 109% method (saddle height = inseam × 1.09) is a useful cross-check.

Cleat position — float, fore-aft, and Q-factor
Cleats do three jobs: locate the ball of the foot over the pedal spindle (fore-aft), set the angular position (heel in or out — match your natural walking angle), and provide rotational float (typically 0° fixed, 4.5°, or 9°). Most knee pain sufferers benefit from at least 4.5° of float — it lets the knee track naturally instead of fighting a fixed cleat. Zero-float cleats are a pro preference, not a default.
Fore-aft: the ball of the foot (first metatarsal head) should sit slightly behind or directly over the pedal axle — a plumb line from the tibial tuberosity (the bump below the kneecap) lands just behind the axle. Too far forward shifts load to the quadriceps and anterior knee. Q-factor (stance width) matters too: too narrow and the knee collapses inward at the top of the stroke; too wide and the medial knee gets stretched. A 2mm spacer per side is often enough to resolve medial tracking issues.

Load management and reloading
Even with perfect fit, doing too much too fast causes knee pain. Classic triggers: a 50% jump in weekly volume, a hilly ride after only flat training, a new set of low-cadence intervals, or a sudden switch to harder gears. The 10% rule (increase weekly volume by no more than 10%) is conservative but safe. When pain starts: back off 50% for 3–5 days, address the fit, then reload gradually — 60% of previous volume for week one, 80% for week two, 100% for week three. Cadence matters: spinning 90+ rpm reduces peak knee forces compared to mashing at 70 rpm in a big gear.
