Cleat fore/aft position
The fore/aft position (how far forward or back the cleat sits under the shoe) affects power transfer and comfort. The traditional starting point: align the cleat so the pedal axle is directly under the ball of the foot (the head of the first metatarsal — the bony bump behind your big toe joint).
The modern approach: many fitters now recommend a slightly rearward position (5–8mm behind the ball of the foot). This reduces calf strain (the calf works less to stabilize the ankle), reduces hot foot (pressure is spread over a larger area), and improves endurance (less muscle fatigue). Sprinters may prefer the traditional forward position for maximal acceleration; endurance riders benefit from the rearward position.
How to find the ball of your foot: feel for the bony bump on the inside of your foot, behind the big toe joint. Mark it with a dot (through the shoe — use a marker on the sole). Align the cleat so the pedal axle (center of the cleat) sits at or slightly behind this mark.

Cleat angle and rotation
Your feet have a natural resting angle — most people's feet point slightly outward (duck-footed) or slightly inward (pigeon-toed). Forcing your feet into a straight-ahead position (which most cleats default to) fights your anatomy and causes knee tracking problems.
Set the cleat angle so your foot sits in its natural position on the pedal. Sit on a table with your legs hanging relaxed — the angle your feet point is your natural stance. Set the cleats so your feet rest at that same angle on the pedals. If you're significantly duck-footed or pigeon-toed, you may need cleat wedges (small shims that angle the cleat) or special cleats with built-in float (SPD-SL with 6° float, Speedplay with adjustable float).
Float: the amount of rotational play the cleat allows before releasing. More float (6°+) is better for riders with knee issues or unusual foot angles. Less float (0–2°) feels more direct and secure.
