The brake lever itself swings sideways to shift to a bigger sprocket or bigger chainring, while a smaller paddle tucked behind the brake lever shifts the other way, to a smaller sprocket or smaller chainring. Both controls sit within reach of the fingers wrapped around the hood.
The genius of STI was integration. Before it, gear shifters lived on the down tube and you took a hand off the bar to shift. STI put braking and shifting in the same place, letting riders change gear while sprinting, climbing out of the saddle, or descending. Every modern road shifter descends from this idea.

Ergopower: Campagnolo's Approach
Campagnolo's Ergopower is the Italian answer to STI and works on the same integration principle but with a different control layout. The brake lever swings sideways to shift up — to a bigger sprocket or bigger chainring — while a thumb-operated paddle on the inside of the lever body shifts back down.
The advantage Campagnolo claims is that shifting up (the heavier cable pull) happens with the whole hand wrapped around the brake lever, while the thumb handles the easier release. The disadvantage is that the thumb paddle is harder to reach from the drops than from the hoods. Ergopower is beloved by Campagnolo loyalists and almost invisible outside that camp.

DoubleTap: SRAM's Single Paddle
SRAM's road shifters use a single paddle behind the brake lever that handles both directions. A short click of the paddle shifts one way; a longer sweep of the same paddle shifts the other. There is no thumb lever and no second paddle — one control does both jobs.
DoubleTap is mechanically simpler than STI or Ergopower, and many riders find it more intuitive once they're used to the travel distance. The trade-off is that accidental over-shifts are easier to make, because a paddle that does both jobs must be moved carefully to hit just the right click.

Di2 and Electronic Shifting
Di2 — Shimano's Digital Integrated Intelligence — replaces the steel shift cable with a wire and a small servo motor at each derailleur. Pressing the shifter button sends a signal; the servo moves the derailleur exactly the right amount. No cable stretch, no housing friction, no misalignment from contamination, no slow drift over a season.
The advantages are tangible. Shifts are identical every time, the front derailleur trims itself automatically as you move across the cassette, and the system can shift under load more reliably than any cable system. The cost is weight, money, and the need to charge a battery periodically.
