Know when to replace
Chains do not fail — they elongate. As the pins and rollers wear, each link's pitch grows longer than the 12.7 mm (half-inch) the cassette was cut for. The cassette teeth then reshape to fit the stretched chain, and once that reshaping is established a new chain will not mesh cleanly with the old cog teeth. The window to catch this is narrow: a chain at 0.5% wear has usually not yet damaged the cassette, while one at 0.75% almost certainly has.
The honest way to measure wear is with a dedicated chain checker that drops into the rollers under spring load. The drop-in type (Park Tool CC-4, Pedros) is more accurate than the old hook-and-pin gauges, which themselves contribute to false readings by levering on the rollers. Ruler-over-12-inches works in principle but is unreadable to better than 0.5% by eye.

Tools you actually need
The minimum kit for a chain and cassette swap is four tools: a chain checker, a chain breaker (or quick-link pliers if your chain uses one), a cassette lockring tool, and a chain whip. Add a torque wrench for the lockring (40 Nm is typical) and a small blob of grease for the freehub splines.
Quick-link pliers are not strictly required — you can break a chain at any pin with a chain breaker — but most modern 10/11/12-speed chains use a quick link, and reusing a broken pin on a 10-speed or narrower chain is a recipe for a weak spot. Get the pliers; they cost ten dollars and save twenty minutes.

Swapping the cassette
Shift to the smallest cog, remove the rear wheel, and remove the quick-release or thru-axle. Hold the cassette stationary with the chain whip on a middle cog, turn the lockring tool counter-clockwise with a wrench, and the lockring unscrews. The cassette then lifts off the freehub body as a stack of cogs and spacers — lay them out in order on the bench.
Before installing the new cassette, wipe the freehub splines clean and apply a thin film of grease. This prevents the cassette from gouging the alloy body under load (the tell-tale notch you find when removing a cassette that has been on for years). Slide the new cassette on — there is only one spline gap that aligns, so if it does not seat, rotate it — and torque the lockring to spec, usually 40 Nm.

Sizing and installing the new chain
New chains come long — sized for the biggest possible cassette — and must be cut to length. The standard method is to route the chain over the big ring and big cog, bypassing the derailleur, pull it tight, and add one full link (inner + outer) to that measurement. This gives enough slack for the derailleur cage to absorb chain growth without binding in the big-big combo.
For 11- and 12-speed drivetrains with clutched rear derailleurs, follow the manufacturer's link count exactly if it is printed on the box — they have done the math for the cage length. Run the chain through the derailleur, install the quick link, and seat it by pedalling hard against the brake with the link on the top run. A new quick link is single-use on most 11/12-speed chains; do not re-install a used one.
